Fees

A 1.5 hour initial evaluation is $300.
A 60 minute hour individual session is $165.
Payment is due at the time of session.

Karen Bernardes LCSW is not an in-network provider with any HMO, PPO, EPO private insurance plan, Medicare or Medi-Cal. 

If you have a PPO insurance policy you will be provided a superbill. You will submit this for reimbursement when using Out of Network benefits.
Please contact your insurance company for their submission process, deductible and reimbursement rates on your PPO plan.  EPO, HMO, Medicare and Medi-Cal plans do not have out of network benefits.

If you are unable to keep a scheduled appointment, you must contact the office by phone or email at least 24 hours prior to your appointment. 

Another appointment (if available) will be offered to you that may work better for your schedule.
If not notified about the cancellation within the time stated above, a full session fee will be charged to the credit card information on file.  

3 missed/cancelled appointments, even if not consecutive, may result in an administrative discharge.

In the event that the payment for services isn’t made at the time of the session or not cancelling in advance as stipulated in the missed/cancelled appointment policy, the credit card information provided at the time of registration will be used to process the balance due. The receipt and statement of services rendered/cancellation fee will be given at the next session or sent to the email/physical address, client approve, on file.
